Job Summary
Position, as part of the primary care team, identifies, triages and manages patients with medical and behavioral health problems within the primary care setting. In addition, will provide skill training through psycho-education and patient education strategies and will consult with primary care providers to develop specific behavioral change plans for patients and behavioral health protocols for target populations. Position provides support and assistance in the development of programs to decrease higher incidence of specific health problems within the farm worker community.
Supervisory and Other Relationships
The position reports to the Director of Operations- Quincy. This position is responsible for the direct supervision of the Promotores staff, employed and volunteer.
Examples of Essential Duties and Accountabilities
The following examples of duties and accountabilities illustrate the general range of tasks assigned to the position but are not intended to define the limits of the required duties. Other essential duties may be assigned consistent with the general scope of the position.
- Work with primary care providers, in a consultative manner, to recognize, treat and manage mental disorders, psychosocial problems, and chronic emotional and/or health problems efficiently and effectively or to refer cases to mental health specialist as appropriate.
- Provide quick and accurate clinical assessments of mental and behavioral conditions, through brief patient contacts, including via telephone and teleconferencing.
- Assist in the detection of “at risk” patients and development of plans to prevent further psychological or physical deterioration.
- Assist in preventing relapse or morbidity in conditions that tend to recur over time.
- To facilitate linkage between agencies, clinic staff and clients.
- Provide advocacy, assessment counseling and prevention education to high risk populations.
- Promote client self-sufficiency through supportive counseling, education on coping, affect, emotional barriers, self-awareness, self-efficacy and self esteem using the Promotores program.
- Community based organizing within the Migrant stream (nationally and bi-nationally).
- Assist in the reduction of access barriers for the farm worker community.
- Maintain and utilize data and resources to effectively communicate and provide culturally competent healthcare services.
- Maintain accurate and appropriate patient records in EMR.
Qualifications
Incumbents are required to have demonstrated basic knowledge and abilities in these areas:
- Bilingual English/Spanish.
- Ability to meet and maintain Mental Health Professional standards of the Washington Administrative Code (WAC).
- Considerable knowledge of behavioral medicine and evidence-based treatments for medical and mental health conditions.
- Good knowledge of psycho-pharmacology and the problems and dynamics in treating medically underserved populations.
- Ability to make quick and accurate clinical assessments of mental and behavioral conditions.
- Ability to design effective psycho-education curricula and lead effective psycho-education classes.
- Ability to work effectively as a team, interfacing with patients, primary care providers, consulting psychiatrist and mental health specialists, as well as with administrative and support staff.
- Ability to exercise balanced judgment in evaluating situations and making decisions, and to handle difficult or confrontational situations in a calm, consistent, and equitable manner.
- Ability to effectively communicate with patients, staff, and the public.
- Ability to communicate in person and on the phone.
- Excellent computer and organizational skills.
These skills and abilities typically are required through a combination of experience and training, which would include a minimum Masters degree in Psychology, Counseling, Social Work or related field, plus three (3) years relevant experience, or a combination of relevant training and experience which would provide the skills needed. Requires a current Washington State Mental Health Counselor Associate or Mental Health Counselor licensure.
